What should I bring with me?

Please print and fill out the appropriate patient forms prior to your appointment, and bring any previously done imaging such as MRI and X-ray images.

How do I get a refill of my medication?

Call your pharmacy and have them fax a request to our office at (801) 314-5112.

Can I get MRI or lab results explained over the phone?

You must schedule an appointment to have Dr. Bowen, Dr. Hurst, or Dr. Clements explain your results.

Will my insurance cover procedures done in this office?

Insurance coverage varies; it's your responsibility to contact your provider to inquire about coverage. Medicaid does not cover procedures in this office.
