Proxy access allows parents, legal guardians, or caregivers to view a patient’s health information through the patient portal. It is available for both adult and pediatric patients.

How do I gain proxy access?

To gain access, complete the 'Portal Proxy Registration Form' for the appropriate family member and return it in person to St. Luke Community Healthcare’s Health Information Management Department.

What happens when my child turns 12?

Proxy access is revoked for patients aged 12-17 in compliance with Montana state law. Parents can request medical records directly instead.

Can divorced parents have access?

Yes, both legally-recognized parents have equal rights to access their children's medical information unless a court order states otherwise.

How can I revoke proxy access?

You can revoke access at any time by contacting St. Luke’s Health Information Management department.
