Can I get my blood type?

We seldom have blood types available, as they are not routinely done unless you have had surgery with significant blood loss. If you had a baby at PMHC, you would have received a card with the baby's blood type.

How can I obtain copies of my medical records?

You must sign a Release of Information form to release records. Parents or legal guardians must sign for minors or mentally incompetent patients. It takes at least 24 hours and up to 30 days to release information.

Is there a charge for medical records?

Yes, we charge $0.25 per page for personal medical records and $1.25 per page for records on microfilm (before 2000). Records can be sent to another physician/facility at no charge if medically necessary.

I have insurance; why did I get a bill?

You will receive a bill after your insurance processes that claim based on what your insurance relays on the Explanation of Benefits (EOB). You will also receive an EOB from your insurance.

What is a co-payment?

A co-payment is a fixed fee you pay at the time of service applied to various visits, including ER and doctor visits.

What is a deductible?

A deductible is the amount you pay before your insurance starts covering services.

What is co-insurance?

Co-insurance is the percentage of covered services you pay after your deductibles are met.

What are your business hours?

Our business hours are Monday through Thursday from 8:00 AM to 4:30 PM, and Friday from 8:00 AM to 4:00 PM.
